In our program, we do not complete milestones for every rotation. We have 3 month blocks and there is an evaluation (Using OrthoBullets Eval Master) midway through the rotation and a final rotation evaluation based on core competencies at the end of the rotation using New Innovations. The CCC discusses each resident at twice yearly meetings, using all the evaluations we have collected during the previous 6 months, and using OrthoBullets determine if the milestones generated by that program are appropriate for each resident. In the end, OrthoBullets populates the milestones for us that we use to complete the milestones twice per year with any information acquired by our quarterly evaluations considered as well. So, my answer is that you can just use the milestones twice per year, but you still have to have other evaluations twice per rotation.
Anyone else have a plan to help out David? Thoughts?

Our milestones are populated using OrthoBullets PASS (https://www.orthobullets.com/anatomy/12103/pass). Residents send pass/fail evals to faculty for surgery cases regularly, complete quizzes and questions, and are assessed by peers and faculty for professionalism. All this data goes into their individual ACGME milestones and then we evaluate each resident at the CCC meeting. It has made the CCC meeting shorter, more concise, and the committee members so happy. It has been a learning curve for all of us but at the end of the day, it is so worth it!

Laurel LewisKeymasterI was lazy. I contacted a company who sent goodies and branded items from my town and state (little known fact: York County, PA is the Snack Capital of the World. Seriously: http://www.phmc.state.pa.us/portal/communities/pa-heritage/snackin-pennsylvania-style.html ). The interns told me they liked it. I just had to send the addresses to this local catering company and if I wanted any residency branded stuff I had to get that to them.

I think it has to be a culture change started by the PD (and Faculty) and then implemented and pushed by your chiefs. Pulling regular reports can be time consuming but it could be a way to help the chiefs support the agenda regulated by the PD. This kind of change is always a struggle!!!
We keep tabs on our residents’ case logs and review with them every quarter, so that is their motivation. Sometimes, they even text me to ask that I not pull their info until last so they can log more cases. I guess I’m just lucky, but my chiefs do put their feet to the fire, with the support of the PD. I am, however, not above guilt trips, bargaining, and holding back reimbursements when ignored in other matters!
