Hi Maggie! I attached the self-reflection we use for the 6-month peds ortho surgery rotation here at Nationwide Children’s. At the three-month mark, the site PDs meet with the residents individually. This is the self-reflection instrument we use. NOTE: in 2026, we are adding an “effort” area as well where we will ask them to rate the effort they are putting forth in the rotation (i.e. not all of them are that “into peds” so it’s reflected in their effort!). It’s not perfect, but it is helping guide the mid-rotation evaluation meeting.
