Self-evaluation and/or semi-evaluation forms

Does anyone have a semi-evaluation and/or self-evaluation form/template that they’d be willing to share?

Hi Maggie!  I attached the self-reflection we use for the 6-month peds ortho surgery rotation here at Nationwide Children’s.  At the three-month mark, the site PDs meet with the residents individually.  This is the self-reflection instrument we use.  NOTE:  in 2026, we are adding an “effort” area as well where we will ask them to rate the effort they are putting forth in the rotation (i.e. not all of them are that “into peds” so it’s reflected in their effort!).  It’s not perfect, but it is helping guide the mid-rotation evaluation meeting.

Hi Maggie,

Attached the self-evaluation I use for mid-year / included in their semi-annual evaluations. I assign it through New Innovations.

I do use these to help me track some of their scholarly activity and pt. safety/QI projects.
