Hi Heather,
I use an electronic form that I created in outlook/microsoft Forms. See the attached, these are the questions I ask and the form is not time consuming for them to complete. I created a QR Code and provided that along with the URL to my residents. They are required to complete this form every month. Per our Sponsoring Institution, I am required to enter their information into MedHub which is our Residency Management System, so the information provided on my form from the residents monthly is essentially transferred to MedHub.

Dawn D’EusanioParticipantHere at Cleveland Clinic South Pointe Hospital, all incoming residents complete ACLS/BLS during orientation. Residents are no longer required to have ATLS Certification.

PGY2’s complete 8 weeks of Ortho Peds at our Sponsoring Institution/main campus
PGY3’s complete 8 weeks at a Children’s Hospital/required out rotation
PGY4’s complete the last 8 weeks back at our Sponsoring Institution
