Misty HansenParticipantHello all, I am hoping someone can help point me in the right direction as to how we should be obtaining patient quality evaluations? Int he past we have printed them and handing them out to patients after surgery (that didn’t go very well) now they are trying QR codes but have found that it is hard to keep the information HIPPA compliant that way.
Any suggestion would be helpful.

Lisa HernandezParticipantHi Misty,
I would be interested in any comments as well. We also tried paper evaluations in clinic/OR that is not consistent and cannot get the staff on board.
I created QR codes for resident badges but we cannot get any feedback.

Maia KnoxKeymasterI was going to comment to partner with your Faculty’s medical assistants/clinic rooming staff and have them do it in clinic, but I see you have tried that already? I know that Fellowship is totally different-I only have 3 fellows and 9 faculty-much easier to handle. But what I do is I give the MAs a paper copy of the Patient Evaluation of the Fellow about a month before they are due and hope they can ask a patient to complete it on their way out. The eval is only 6 questions long, so it should take maybe 2 minutes to complete, which makes it more likely that a patient would take the time. I am surprised that QR Codes have not worked? You both have given me the idea to do that instead myself… I should mention that I do have to remind the MAs maybe twice during that month… Sorry I am not of much help, but I wanted to comment anyways…

Misty HansenParticipantI think the issue with QR codes is that the patient has to complete a handful of steps before they get to the evaluations. At least for New innovations they do.

Kim ReisingParticipantWe struggle with this as well. For anyone who has had a site visit within the last couple of years, has anyone been dinged or received a citation for not having completed evaluations from the patients?

Lisa HernandezParticipantWe did the QR codes through Canvas an Institutional license, just took you straight to the fill in form.

Courtney MatlachKeymasterHi Kim-
I had a site visit in May of 2024 and we were asked to provide a “multi-source evaluation of a resident/fellow by peer, self, nurse, non-physician staff member, or patient”. So we actually submitted an evaluation from a nurse, not a patient, and it satisfied the requirement.
Thanks,
Courtney

Laura BrownKeymasterI create a survey in Microsoft Forms. It creates a link and a QR code, and I print that QR code out and have the clinic display it. I’ve thought about QR codes on a badge placard before. It’s always been tricky to get more than a few. I think the concept of a QR code is still tricky for some patients, or feels more daunting than a piece of paper.
I attached an example of the QR code/survey.

Lisa HernandezParticipantThank you, Laura. We tried that but with 20 residents we think that is why patients stop scrolling to find the resident.
I think if the survey is anonymous with no patient information using google forms or Microsoft forms should be compliant. UF being a safety net hospital makes it quite challenging.
I think my best avenue is the QR on the resident badge and just enforce they should get a minimum of 1 a rotation.
Thanks, Courtney, for the site visit information- that is comforting. My institution is very adamant on patient evals.
