Hi everyone,

Checking in to see if anyone has had a site visit recently and willing to share their evaluation form that the residents complete on the faculty (that met the approval of the site reviewer and RC).

Hi Kim! I’ve attached a template of the eval we’ve been using. We distribute it and collect responses via SurveyMonkey, so the formatting isn’t great in Word. We had a site visit in Jan 2023 and the visitor had no issue with it.
