    My docs are finding the new Milestones 2.0 tedious and hard to rate rotation by rotation.  The consensus is that submitting Milestone evals in New Innovations block by block is somewhat pointless and it would be better suited to discussing them resident by resident in the CCC meetings.  I’m search ACGME and I don’t see anything that says we have to do milestone evals block by block, just that we have to submit them twice a year.  Am I missing anything?

    In our program, we do not complete milestones for every rotation.  We have 3 month blocks and there is an evaluation (Using OrthoBullets Eval Master) midway through the rotation and a final rotation evaluation based on core competencies at the end of the rotation using New Innovations.  The CCC discusses each resident at twice yearly meetings, using all the evaluations we have collected during the previous 6 months, and using OrthoBullets determine if the milestones generated by that program are appropriate for each resident.  In the end, OrthoBullets populates the milestones for us that we use to complete the milestones twice per year with any information acquired by our quarterly evaluations considered as well.  So, my answer is that you can just use the milestones twice per year, but you still have to have other evaluations twice per rotation.

    Anyone else have a plan to help out David?  Thoughts?

    Hi David,

    We are similar to Laurel’s process.  We do not use milestones as a rotation evaluation tool.  We use Orthobullets Eval Master for end of rotation evaluations.  Even before we used Orthobullets, we had our own evaluation outside of the Milestones for the rotations.  We only do Milestone evaluations every 6 months during the CCC meeting. I hope this helps.

    Here is what I shared with David.

    We only have to report the milestones twice per academic year (mid-year and end of the year), as required by the ACGME milestones reporting.

    How you choose to use them as an assessment tool with rotations, evaluations, CCC meetings, etc. is up to your program.

    Hi David,

    Similar to the others we report the milestones twice a year.  Milestones are competency based grades looking at five years of development. We use traditional achievement based evals in NI to complement the competency based evals. Hope this helps and let us know if you have nay other questions.
