Does anyone have an Fellowship Alumni Survey form that can share with me? We would like feedback from our fellows, we have not done this is the past.

Hi Maria – I apologize I forgot to respond to this sooner, but we did implement a alumni survey of the residency and a fellowship director evaluation of our graduates last year. I’ll upload both of them.
If anyone thinks they look familiar, I borrowed content from other programs as I had asked the same question a few years ago.

Maria: I’ve attached a copy of the alumni survey that we send to our residents. You may be able to use some of the questions for a fellowship alumni survey. I hope it’s helpful.
