Hello!
I am a newer program manager, and I have a few questions about clinic. From what I am reading in the ACGME requirements, my residents need to have at least one-half day a week in clinic. My residents seem to think that the clinic time is averaged over the course of residency, but that isn’t how it is written. Can someone please clarify this for me?
I also noticed the requirements say that they need to see at least 10 patients while at clinic. How are you tracking that? Are you requiring them to log those cases?

These are good questions!
We do not track the resident clinic days or the number of patients, nor do they log the patient experiences. The residents follow faculty who require that to participate in the OR, you also have to be well-rounded and learn how to manage the clinic, as well. Depending upon the service and which year the resident, they may attend 2-4 half day clinics per week, on other rotations substantially less clinic days.
We use our evaluations to check their attendance at clinic, both faculty and residents confirm they are having clinic experiences. You could add an attestation question to your evaluations that asks faculty to confirm days in clinic and if the resident saw 10+ patients per week.
Having said all this, I have 3 residents per year and know where they are depending upon their rotation.
Does anyone have another method to monitor resident clinic attendance and patients encountered?

I use an electronic form that I created in outlook/microsoft Forms. See the attached, these are the questions I ask and the form is not time consuming for them to complete. I created a QR Code and provided that along with the URL to my residents. They are required to complete this form every month. Per our Sponsoring Institution, I am required to enter their information into MedHub which is our Residency Management System, so the information provided on my form from the residents monthly is essentially transferred to MedHub.

The way we record/track it is when logging duty hours, my residents have to specifically log clinic hours, there is a drop down option for this. Not only are we able to ensure and prove they are adhering to ACGME guidelines, which honestly is not an concern for our program, we utilize that feature (clinic hours) for reporting the number of hours/percentage of time residents work to our Finance department for budget control sheet reporting to CMS.
