ACGME Standardized Training Verification

Does anyone use the AHA-ACGME-OPDA-VGMET Fillable form that is posted on ACGME. The number of verifications requested, and the minor differences in multiple forms is aggravating. It would be great to use a standardized version, but I don’t know that it will be accepted.

Our institution came up with forms we are required to use.  It came from the legal department so information provided is limited.  So far, I have had no issues from places requesting verification.

I have used it for a while and only recently had one hospital system refuse to accept it.

I have used it for years now.  The PD completes it at graduation and I save a PDF so it cannot be altered.  Whenever I get a request, I reply with the standard ACGME form.  99% of the time they accept it without issue.

I also see how it will be useful many years down the road when faculty has changed and the PD does not personally know the past resident.

For those of you that use the standard form, how does that work with the electronic verifications that are sent through programs like docusign?  A lot of those are sent with no contact information.
