Just a quick reminder that you must use of one the templates provided by the ACGME for your block diagram for your ADS report.

Is there a link to where this info is at? Or the templates? Our Fellowship ADS account doesn’t state anything about a required block diagram template.

I am looking at the sample block schedule on the annual update and its the same diagram that we used in the past.

Hi Andrea,

I attached it here but you can also find it on the ACGME site under your specialty, on the right menu “Documents & Resources” under the Documents list.

Scroll down to where it says “Orthopaedic Surgery Core Block Diagram”
There seems to be no other instructions but the file says REQUIRED at the end. I did not use this file for my fellowships.

Also, you don’t need to use the updated form for your fellowship but you do need a block diagram for the fellowship . Email me if you have questions: llewis8@wellspan.org
