    Hi all!

We are working on a complement increase and trying to get the information to ACGME by the October deadline. The complement was approved by our GME office, but we are still waiting on financial approval from the university. My DBO asked me if it is possible to submit to the ACGME without financial approval and I am unsure if that is okay.

Also, the DBO said there is a chance that even if we get approved by the ACGME for the complement change in 2027-2028 that we may need to wait until 2028-2029 to implement it. Does anyone know if the ACGME requires us to implement the complement?

My Program Director is against this idea, as it puts us at risk of the same citations we have received before, but without making changes, but wanted to ask before going back to the DBO.

    Shawna,

It would seem the approval process at your institution is different than ours, as we are required to have financial approval before submitting a request for GMEC approval.  Since GMEC approval is required before the request can be submitted to the ACGME, I would start by asking whether your DIO would be willing to sign off on the request without the financial approval in place?   I would guess that they would not, but that is where I would start.

As for the timing of implementing the increase, we were approved for an increase that would have taken effect July 2025 but did not actually implement the increase until July 2026.  I think it’s always best though, to ask the ACGME directly.  Honestly, from my first hand experience, they are not the “big bad wolf”.  They are there to help guide programs and help us succeed.   Again, I would start by asking whether your DIO is willing to sign off on the ACGME request without the financial approval, as that may make the rest of the question moot depending on their response.

    Hi Shawna,

We just sent in our materials for a compliment increase to catch the current RC meeting, too.

I agree with Cindy, if you can get your DIO to sign off, you don’t have to implement the increase until you are ready.  If you think about it, unless you find PGY’s 2-4 in the match, or elsewhere,  you will have open spots in your program for a few years.

Again, agreeing with Cindy, our ACGME contacts have been very communicative since we sent the info in.  I would suggest emailing both Citlali cmeza@acgme.org Associate Executive Director and Deanna DEallonardo@acgme.org Accreditation Administer and pose your questions.

    Hope it works out!

Same as others, we can’t get GME approval without our financial support finalized first.
