    Hello,

    Looking for suggestions on how to distribute patient evaluations of the residents after being seen in clinics. Since many patients are older, we would like to stay away from electronic surveys, codes, etc., rather use a paper form or something similar.

    If there are better alternatives, please let me know as well!

    Hello, so we print them out and put the evals in envelopes and leave it to the responsibility of the learner to get them. We have a policy that they need to get 2 per block (and we have 3 month blocks). Once the evaluation is completed, they put it in my mailbox outside my office. It seems to be working well for us.

    We use a paper evaluation as well; single sheet/one-sided..  I give them to the clinic manager to help distribute to the patients based on resident schedules.
