    Hello everyone!

    We are in the final steps of the process of starting an Ortho PT Residency – Have been accepted for accreditation candidacy and we are preparing to welcome our first class in July. Some of the evaluations needed for this Program are a bit more detailed that what we use for Sports Surgery, so I am struggling a bit with building these accordingly. Do any of you have a ‘Faculty evaluation of PD’ and a ‘PD evaluation of Faculty’ that would share with me?

    I am aware that the Fellow evaluation of Faculty can be used by the PD also to evaluate the Faculty, maybe slightly modified, but it would be a good starting point… But I thought I’d ask if any of you have a good one to share.

    Hi Maia!  Hope you are doing well.

    Attached, please find the evaluation form we use for the PD of Core Faculty.  While we do not have a form specifically tailored for faculty evaluation of PD, our annual Faculty of Program Evaluation may offer some helpful insights, as it includes questions related to both PD and PM.
