We are looking to collect performance feedback on our residency program’s recent graduates by surveying their current fellowship Program Directors.  Do any of you have an example of a survey or evaluation that is sent to a graduated resident’s current fellowship program director in order to evaluate how the program training is preparing residents for fellowships and practice?
