We purchase jackets for our interns with the institution/department logo embroidered on it. Sometimes the department will give away various employee appreciation items during the year and we will save some aside to give to our incoming interns as well. For a tight budget, maybe a custom lanyard? I know our residents and fellows always like new lanyards for badges.
