    Hello,

    Does anyone have a basic case log form they can share for an unaccredited fellow?

    Good afternoon Kelly,

     

    I have 2 unaccredited programs (Recon and Foot and Ankle) and the fields that I use are:

    • Surgery/Case #
    • Date of Service
    • DOB
    • Gender
    • Diagnostic
    • Procedure
    • CPT Code
    •  Level
    • Attending/Surgeon
    • Location

    Hi Kelly,

    I have one I can share with you. If you give me a call we can go over how I have it set up, which codes are tracked, and how the formulas in the spreadhseet work, etc. I’ve tried to attach it here but it might be too large of a file.
