    Hello:

    Would anyone be willing to share there Faculty of Resident evaluation forms.  Also the questions related to SBP, PBLI, PROF and ICS.

    Hi Michelle,

    Ours are rotation specific and different with the new milestones in them. (picked which PC, MK milestones should be for “trauma, peds, etc” and then added the standard SBP, PBL, Prof  & ICS to all of them) I’ve attached what we use for faculty evaluation of residents on the Hand rotation as an example. Not the best PDF 🙂
